The Nursing Process (ADPIE)

The nursing process is a five-step problem-solving framework — Assessment, Diagnosis, Planning, Implementation, and Evaluation (ADPIE) — that guides safe, individualized, patient-centered care.

Overview

The nursing process is the backbone of professional nursing practice. It is a systematic, patient-centered method for thinking through care — a repeatable way to figure out what a patient needs, plan for it, act, and check the results. Its five steps are captured by the acronym ADPIE: Assessment, Diagnosis, Planning, Implementation, and Evaluation. Rather than reacting to problems randomly, the nurse follows this cycle to deliver organized, individualized, and safe care. The process is dynamic and cyclical: findings from evaluation feed back into a new round of assessment.

Step 1: Assessment

Assessment is the deliberate, systematic collection of patient information. Nurses gather two kinds of data:

  • Subjective data — what the patient reports and only the patient can feel, such as pain, nausea, dizziness, or anxiety. These are symptoms.
  • Objective data — measurable, observable facts the nurse gathers, such as vital signs, lab values, skin color, or a wound’s appearance. These are signs.

Data come from many sources: the patient (the primary source), family, the medical record, and the health care team. Good assessment is thorough and accurate because every later step depends on it. A cluster of related findings is then analyzed to identify the patient’s needs.

Step 2: Diagnosis

In the diagnosis step, the nurse analyzes the assessment data and forms a nursing diagnosis — a clinical judgment about the patient’s response to an actual or potential health problem. This is different from a medical diagnosis, which names a disease.

A patient with the medical diagnosis of a stroke, for instance, might have nursing diagnoses of impaired physical mobility, risk for falls, and difficulty swallowing — problems nurses are licensed to treat.

Step 3: Planning

Planning sets priorities and defines goals. Nurses often prioritize using frameworks such as Maslow’s hierarchy — airway, breathing, and circulation before comfort or teaching. For each priority, the nurse writes patient-centered goals and expected outcomes, then selects interventions to reach them.

Effective goals follow the SMART criteria:

  1. Specific — clearly stated
  2. Measurable — quantifiable so progress can be judged
  3. Attainable — realistic for this patient
  4. Relevant — meaningful to the patient’s situation
  5. Time-bound — has a target date or time

“The patient will walk 50 feet in the hallway with a walker by the end of the shift” is a SMART goal; “the patient will walk more” is not.

Step 4: Implementation

Implementation is carrying out the plan — performing or delegating the nursing interventions. This includes direct care (giving medications, repositioning, wound care), teaching, and coordinating with the team. Throughout implementation, the nurse continues to observe the patient’s response and documents each action accurately. Safe implementation means acting within one’s scope of practice, following facility policy, and never omitting the ongoing reassessment that keeps care responsive.

Step 5: Evaluation

Evaluation asks a single essential question: Did we meet the goal? The nurse compares the patient’s current status to the expected outcomes. Goals may be met, partially met, or not met. If a goal is unmet, the nurse does not simply stop — the process loops back: reassess, revise the diagnosis or plan, and try again. This feedback loop is what makes the nursing process a continuous cycle rather than a straight line.

Critical Thinking Throughout

Every step demands critical thinking — disciplined, reflective reasoning. Nurses interpret data, question assumptions, recognize patterns, and adjust as the patient changes. The nursing process gives this thinking a reliable structure so that nothing important is missed.

Clinical Relevance

The nursing process is not an academic exercise; it is used with every patient, every shift. It ensures care is individualized (built from that person’s data), organized (steps in a logical order), and safe (goals are evaluated and revised). It also standardizes communication: when a nurse hands off a patient, describing assessment findings, active nursing diagnoses, the current plan, and evaluation results gives the next caregiver a clear, complete picture. Mastering ADPIE early builds the clinical judgment that underlies all later nursing skills.

How a formal nursing diagnosis is structured

Standardized terminologies such as NANDA-I frame a diagnosis in a PES format: the Problem (the diagnostic label), its Etiology ("related to" the contributing factors), and the Symptoms or defining characteristics ("as evidenced by" the assessment data). For example: impaired physical mobility related to lower-limb weakness as evidenced by inability to bear weight. Writing the etiology precisely matters because interventions target the cause, not just the label.

Prioritization frameworks behind the planning step

When several problems compete, nurses rank them using structured hierarchies rather than intuition alone. Maslow's hierarchy and the ABC mnemonic (airway, breathing, circulation) push life-threatening physiologic needs ahead of comfort, safety, or teaching goals. Contemporary education also maps this reasoning onto the NCSBN Clinical Judgment Measurement Model, which breaks decision-making into recognizing and analyzing cues, prioritizing hypotheses, generating solutions, taking action, and evaluating outcomes.
